Admission Requirements:

  1. Please ensure that you have a softcopy of the following requirements to be uploaded before proceeding:
    1. 2x2 picture in white background
    2. Clear copy of grades
      1. Grade 11 card for graduating Senior High School
      2. Grade 12 for graduates of Senior High School
      3. Certification of Grades of Transcript of Records for Transferees
      4. Certificate of Rating with remarks indicating qualification for college level admission for ALS graduate
    3. Other Relevant ID / Certification, if applicable
      1. Persons with Disablities (PWD)
      2. Indigenous People (IP)
      3. Solo Parents
      4. Out of School Youth (OSY)
      5. 4 Ps Beneficiary
  2. Only ONE application should be filed for the whole University System. Multiple submission of application can be a ground for disqualification.
Note: Some academic programs may have other officially approved requirements prior to admission. If you wish to change any part of the application, contact the campus where application is submitted.
