DATA PRIVACY CONSENT FOR APPLICANTS OF PSU COLLEGE ADMISSION

1. By providing information to the Admission Office, I am confirming that all data supplied and uploaded are true and correct. I understand that giving false information will make me ineligible for admission and that PSU reserves the right to revise any decision made on the basis of the information I have provided, should the information be found to be untrue and incorrect.
2. I understand that by applying for admission at PSU, I give permission to the University to collect, use and further process information on personal circumstances and directory information such as but not limited to address, mobile number, family data, academic information/background, and other demographic profile.
3. I am aware that said information will be accessed and used by the Admissions Office and by the other school personnel who are involved in the evaluation of application for admission.
4. I also give my consent to use my data in formulating policies to further improve the University's admission process.
5. I understand that copy of my application and photocopy of other personal documents will not be returned to the applicant and will be properly disposed by the institution if not accepted or if I decide not to pursue the application.
6. I also understand that the data I have provided will be submitted to other office/agency such as CHED and UniFAST for other purposes related to admission.
