DATA PRIVACY CONSENT FOR APPLICANTS OF PSU COLLEGE ADMISSION

1. I certify that all information supplied in the application are true and correct. I understand that giving false information will make me ineligible for admission and that PSU reserves the right to revise any decision made on the basis of the information I have provided, should the information be found untrue and incorrect.
2. Before submitting my application for admission, I have read, understood and give consent to the University on the collection and use of supplied information such as but not limited to name, address, contact numbers, family data, and educational background.
3. I am aware that the said information will be accessed and used by the Admission Office and by other school personnel who are involved in the evaluation of application for admission. Hence, I agree to the posting of my name in the official FB page of PSU for ease of communicating admission results.
4. I also give my consent on the use my data in formulating policies to further improve the University's admission process.
5. I understand that copy of my application and photocopy of other personal documents will not be returned to the applicant and will be properly disposed by the institution if not accepted or if I decide not to pursue the application.
6. I also understand that the data I have provided will be submitted to other office/agency such as CHED and UniFAST for other purposes related to admission.
